Colorado Insurance Program

CCRM Fertility of Colorado participates with numerous insurance and fertility benefit providers. Our team is committed to helping our patients understand their insurance coverage and identify any potential gaps in coverage that may exist. We also partner with Future Family to offer financing options and offer multi-cycle packages to help make treatment more affordable.

Below is our list of contracted insurance carriers. We will bill your primary insurance carrier only if you have IVF insurance coverage and/or fertility insurance coverage and we participate with your insurance carrier and plan. Please note that we are out-of-network with all HMO plans and that we will only bill your primary insurance carrier. We can bill your services under your partner’s policy only if you are an active member on their policy and it is your primary insurance.

PARTIALLY IN-NETWORK INSURANCE PLANS

Insurance plans that are partially in-network with CCRM only cover physician consults, in-office procedures, and ultrasounds. Partially in-network plans DO NOT cover services provided by Fertility Labs of Colorado, CCRM’s IVF Lab and Surgery Center, which include blood work, semen testing, IVF/Embryology procedures, and Surgery Center fees.

If your insurance carrier or plan is out-of-network with us, payment is expected at the time of service. If you wish to submit services to an out-of-network insurance or secondary insurance, we are happy to supply you with an itemized receipt upon your request for you to submit to your insurance company.

CCRM is committed to ensuring you fully understand any insurance coverage that you may have, as well as any potential out-of-pocket costs associated with your treatment plan. CCRM Fertility of Colorado also accepts Maven benefits. Maven partners with employers to provide additional coverage and support for fertility services not often covered by your commercial insurance plan. Check with your employer to determine if Maven is a benefit option.
