Your First
Appointment

Prior to your new patient consultation, you’ll need to complete your new patient paperwork and send us your medical records, including any previous fertility tests and treatments, annual gynecological exams, and immunization records. Having this information ahead of your appointment helps us get familiar with your unique health history, which expedites a smooth treatment process, and allows for us to dive more deeply when we first meet.

During your initial consultation, we’ll discuss your personal health history, as well as your fertility concerns and goals. Your CCRM Fertility doctor may recommend personalized testing as a next step to evaluate your fertility and develop a care plan that’s tailored to your specific needs.
