After you’ve scheduled your telehealth appointment, check your email for a welcome letter with new patient paperwork and instructions on how to log into your appointment. If you don’t see it in your inbox, check your spam or junk folder.

Your First

Prior to your new patient consultation, you’ll need to complete your new patient paperwork and send us your medical records, including any previous fertility tests and treatments, annual gynecological exams, and immunization records. Having this information ahead of your appointment helps us get familiar with your unique health history, which expedites a smooth treatment process, and allows for us to dive more deeply when we first meet.
