Intrauterine insemination (IUI) is a common fertility treatment option that involves placing washed sperm directly into the uterus to increase the chances of fertilization. Our IUI Process at CCRM Fertility

While IUI continues to be the gold standard of infertility treatment in the medical community, at CCRM Fertility of San Francisco, we understand that every patient is unique. That’s why we begin the IUI process with a comprehensive evaluation of both partners, including a thorough medical history, physical examination, and laboratory testing. Our experienced team of reproductive specialists will then collaborate with you to develop a personalized treatment plan that addresses your specific needs and goals.

Before beginning IUI treatment, patients may undergo ovulation induction to stimulate the ovaries and increase the chances of successful fertilization. This may include taking oral medications or receiving injectable hormones, depending on your individual needs.

Once the ovaries are stimulated, we will monitor your cycle closely using ultrasound imaging and hormone testing. When the follicles have reached the appropriate size, we will administer a trigger shot of human chorionic gonadotropin (hCG) to trigger ovulation.

Approximately 24-36 hours after receiving the hCG shot, you will return to our clinic for the IUI procedure. During the procedure, we will insert a thin catheter through the cervix and into the uterus, where washed sperm will be injected directly into the uterus. The procedure typically takes less than 30 minutes, and patients can return to normal activities immediately afterward.

Who are Ideal Candidates for Intrauterine Insemination?

Ideal candidates for intrauterine insemination include:

  • Couples who have been trying to conceive for at least six months without success
  • Couples using donor sperm or same-sex couples who wish to conceive
  • Couples experiencing male infertility, such as low sperm count or decreased sperm motility
  • Women who have ovulation problems, hormonal imbalances, or other fertility issues, for whom IUI may be recommended as a first-line treatment option.

What are the Requirements for IUI?

To receive IUI treatment at CCRM Fertility of San Francisco, patients must meet the following criteria:

  • Undergo a comprehensive evaluation to determine the underlying cause of infertility, which may include blood tests, semen analysis, and other diagnostic tests
  • Ovulate regularly, or be able to undergo ovulation induction
  • Have open and unblocked fallopian tubes
  • The male partner must have a sufficient sperm count and motility

Is IUI Covered by Insurance in California?

If you’re considering intrauterine insemination (IUI) as part of your family-building journey, it’s important to understand how insurance coverage may support your treatment. In California, fertility coverage varies depending on your plan type and provider, but recent legislation is helping expand access for more individuals and couples, including those in the LGBTQIA+ community and single parents by choice.

While IUI is often one of the first steps in fertility care, coverage is not guaranteed under every plan. For patients enrolled in fully insured large group health plans (employers with more than 100 employees), California law requires coverage for the diagnosis and treatment of infertility beginning early 2026. This may include IUI when deemed medically necessary.

Small group plans (100 or fewer employees) must offer infertility coverage, but IUI may not be included unless the employer opts to provide it. Self-insured employers, religious organizations, and Medi-Cal managed care plans are not required to follow state fertility mandates.

IUI may be covered in cases such as:

  • Mild male factor infertility
  • LGBTQIA+ family building using donor sperm
  • Single individuals using donor sperm
