Houston, TX fertility insurance program
CCRM Fertility of Houston participates with numerous insurance and fertility benefit providers. Our team is committed to helping our patients understand their insurance coverage and identify any potential gaps in coverage that may exist. We also partner with Future Family to offer financing options and offer multi-cycle packages to help make treatment more affordable.
Texas state law requires certain insurers that cover pregnancy services to offer coverage for in vitro fertilization (IVF insurance benefits). That means insurers must let employers know this coverage is available. However, the law does not require those insurers to provide the coverage; nor does it force employers to include it in their health plans. The law does not require organizations that are affiliated with religious groups to cover treatment that conflicts with the organization’s religious and ethical beliefs. (Texas Insurance Code, Article 3.51-6).

Partially in-network insurance plans
Insurance plans that are partially in-network with CCRM only cover physician consults, in-office procedures, and ultrasounds. Partially in-network plans DO NOT cover services provided by Fertility Labs of Colorado, CCRM’s IVF Lab and Surgery Center, which include blood work, semen testing, IVF/Embryology procedures, and Surgery Center fees.

We know that IVF and other fertility treatment costs can be a major source of stress during the fertility treatment process. We will work hard to help you be informed about your insurance coverage and other financing options.
If your insurance carrier or plan is out-of-network with us, payment is expected at the time of service. If you wish to submit services to an out-of-network insurance or secondary insurance, we will be happy to supply you with an itemized receipt for you to submit to your insurance company.
CCRM Fertility of Houston is proud to partner with Future Family, which offers affordable IVF and egg freezing plans, concierge care, and on-demand medical support. 0% interest financing is available on fertility treatments for up to 12 months, exclusively for eligible CCRM patients.
CCRM Fertility of Houston proudly accepts Maven benefits. Maven partners with employers to provide additional coverage and support for fertility services not often covered by your commercial insurance plan. Check with your employer to determine if Maven is a benefit option.

CCRM Fertility of Houston also accepts Progyny benefits. Progyny covered services may vary based on the employer sponsoring the benefit plan. All Progyny patients (new or existing at a clinic) must first call their Progyny Patient Care Advocate (PCA) for authorization before scheduling an appointment with our clinic.

Fertility insurance coverage in Texas: what you need to know
At CCRM Fertility of Houston, we understand that navigating insurance coverage for fertility care can feel overwhelming. That’s why our team is here to help you understand your options and guide you through the process—from verifying your benefits to exploring financing solutions.
Texas law requires insurance providers to offer infertility coverage to group health plans, but it’s up to individual employers to decide whether to include those benefits. This means that coverage for treatments like IVF, IUI, or fertility preservation can vary widely depending on your plan.
If your employer-sponsored plan includes infertility coverage, you may qualify based on certain medical criteria. These typically include:
✔ A history of infertility lasting at least five consecutive years
✔ A diagnosis related to conditions such as endometriosis, blocked fallopian tubes, or low sperm count
✔ Prior attempts at less costly fertility treatments that were unsuccessful
✔ Use of your own egg and your spouse’s sperm, per Texas requirements
✔ Treatment performed at a qualified facility that meets ASRM and ACOG standards—like CCRM Fertility of Houston
Fertility preservation coverage
Under Texas law, insurance plans must cover fertility preservation services when a patient is undergoing medically necessary cancer treatment (such as surgery, chemotherapy, or radiation) that could impact fertility. This includes the collection and preservation of:
✔ Eggs
✔ Sperm
✔ Ovarian tissue
Please note: Storage of reproductive materials is not required to be covered under current Texas law.
Expectations to coverage
Some plans and providers are not required to include fertility benefits, including:
❌ Employers who self-insure
❌ Religious organizations that opt out based on belief systems
❌ Medicaid managed care plans, which are not required to cover fertility preservation
