What insurance does CCRM accept?
Plans update and change routinely. Please contact your CCRM clinic to verify coverage prior to your scheduled appointment. Some insurance plans will require a referral from your primary care provider (for example an OB/GYN) before receiving treatment for fertility services. It is your responsibility as the patient to provide CCRM with a copy of the referral prior to your scheduled appointment.
How will I know if a referral or prior authorization is needed?
If you have an HMO plan, a referral will be required. For other insurance types, your financial counselor will review your plan details and let you know if any prior authorizations or referrals are necessary for your treatment.
Will my insurance cover fertility treatment?
Coverage for fertility care varies by insurance plan (and by state). After your treatment plan is finalized, your financial counselor will walk you through your options, including any available benefits, expected out-of-pocket costs, authorizations or deductibles that may be required, and payment timelines. Please note that treatment costs are typically due before the start of your cycle.
How can I confirm my insurance coverage and benefits?
If you have employer-sponsored health insurance, talk to your HR rep or review your insurance policy to see what may or may not be covered. Also consider calling your insurance company directly to find out what fertility benefits you may be entitled to.
How do I talk to my employer about providing fertility benefits?
To advocate for fertility benefits at work, set up a direct meeting with HR. Come prepared with information on how fertility coverage supports employee well-being, improves retention, and promotes a more inclusive workplace. Strengthen your case by gathering support from colleagues who may also benefit from expanded coverage.
