Information on payments made to the facility for defined bundles of services and procedures is available at https://price.healthfinder.fl.gov/#!. The service bundle information is a non-personalized estimate of costs that may be incurred by the patient for anticipated services, and actual costs will vary based on services actually provided to the patient.
Financial disclosure
Currently, CCRM Florida Surgery does not participate with any insurance carriers. At this time, all fees are self-pay and payment is expected prior to services rendered.
Charges
Charges for the facility, physician, and anesthesia services are separate. If applicable, you will be billed separately for services rendered by your physician(s), anesthesiologist, and pathology.
Payment
All out of pocket costs must be paid in full prior to services rendered. You may pay by cash, check or credit card. For financing options, please speak with your financial counselor.
Collections
For outstanding balances, any account that is more than 90 days past due may be reviewed and referred for collections. If the account is referred for collections, a collection fee of $20 will be assessed. Additionally, the patient may also be liable for costs associated with collection efforts, including court costs and reasonable attorney fees.
Discounts
CCRM Florida Surgery offers discounts for veterans, active-duty military, and oncology patients. For additional information or to see if you qualify, please speak with your financial counselor.
